The Education Quality and Accountability Office (EQAO) is a Canadian government agency that was formed by the Ontario government in 1996 with the goal of ensuring that all students were being assessed according to an estabilshed set of standards and to hold the province's publicly funded education system accountable to its goals.

Since its inception, the EQAO writes a yearly series of Mathematics and Language Arts assessments for Grades 3, 6, and 9. The Grade 9 Language test (Ontario Secondary School Literacy Test) is a requirement for an Ontario Secondary School Diploma.
